When we receive a valid Authority to Release Information form (pdf), we'll automatically send you a Client Summary upon the authority being accepted, generally within 3 business days. If you already have a current and valid authority and want an updated Client Summary, complete the form below.
If your client holds an Income Account, a Centrelink Schedule will automatically be included at the end of the Income Account Client Summary.
